We use a specialized Electronic Medical Record system so our patient files can be readily accessed at any of our locations.
If you require your medical records to be transferred to another physician, please send a signed release to contact@nduc.org or fax it to 216-273-7013. We will require a signed release before we can process your request. Please keep in mind there will be a charge for this transfer and will be based on the quantity of pages requested. If you wish to view the original record, one of our staff must be present to maintain the integrity of the record.
Medicaid and Medicare Coverage: NextDoor Urgent Care is contracted with all Ohio Medicaid and Medicare plans. Patients will need to provide their insurance card and photo ID. All copays are due at the time of visit.
Test Results and Consultations: Results can’t be given over the phone. Meet our providers in person to discuss and understand your test outcomes.
Prescription Policy: Our providers don’t prescribe narcotics for patients of other physicians. We prioritize safe and responsible healthcare practices.
Work-related Injuries: If you’re visiting due to a work-related injury through The Bureau of Workers’ Compensation (BWC), please remember to bring the completed First Report of Injury Form for seamless assistance.
Follow-up and Test Results: Patients are expected to return for follow-up on test results as necessary. After your test, the facility will inform you of the expected arrival time of results at NextDoor Urgent Care (NextDoor UC). When your results are received, our providers reviews and appropriate tests calls are made for both positive and negative test results.
Out of Network Patients: At NextDoor Urgent Care we believe insurance should not dictate choices for patients regarding their own healthcare choices and work tirelessly to meet our patients needs. Please refer to our Out of Network Policy.
Uninsured Patients: We offer the absolute best rate for uninsured patients to be seen in a timely and compassionate manner. Initial office visit charge of $130 and any further cost for testing or procedures will be discussed with the patient as well as treatment options to help minimize costs to our patients. Our primary concern at NextDoor Urgent Care is your health.
